Help in Hospital

Somerset County Council recognise that when you are in hospital it may be that you, your family or hospital staff feel you will need extra support to get you home swiftly and safely.  If you are linked to a named social worker at that time, then the ward should contact them to help plan your discharge. If not, you will be offered, or can ask for, a referral to Adult Social Care.

The social worker will assess your needs and talk to you, your family and hospital staff. Other assessments may be needed to give a full picture, for example from an occupational therapist, physiotherapist, district nurse or psychiatric nurse. 

Once it is clear what your needs are, the social worker will agree a discharge plan with you and all concerned. This may include some help or support from Adult Social Care or other organisations when you get home.

In the unlikely event that your assessment shows you cannot return home safely, the social worker and others involved will discuss the alternatives with you and your family. The options may include supported housing or a placement in a care home.

It is important that you leave hospital as soon as you no longer need hospital care. Adult Social Care may be able to help with the cost of any services or support you need.

After you have left hospital, Adult Social Care will contact you to make sure your assessed needs are being met, and may increase or reduce any support or services, depending on how well you are managing.
