Somerset County Council manage the Supporting People programme in conjunction with five District Councils (Housing Enablers), Health (Somerset Primary Care Trusts) and Probation. The aim of the programme is to help vulnerable people who are homeless or at risk of homelessness to live independently and to sustain their tenancies.
Services include accomodation based projects such as homeless hostels, womens refuges and sheltered housing, and floating support services which provide support in person's own home. Floating support includes Home Support for older people and those with physical disabilities, family floating support, NOVAS for single people under 65 years old and Rethink for older people with mental health issues.
Support Workers work alongside the service users enabling them to do things for themselves, helping them to adapt to their environment, learn new life-skills or regain confidence in existing skills and access other services such as health, education and advice.The services are for vulnerable people, and will include:-

Young people at risk

Frail older people with support needs

Ex-offenders

People with mental health problems

People with sensory loss, physical or learning disabilities

People with HIV/AIDS
