What We  Write About You Finding Out What We Write About You

We write things about you when you:

  • See someone in the Community Team
  • Go to day services
  • Live in a home with staff to support you

We write down what we talk about. This is to help us remember and think about how we can work together. It also helps staff know how to support you.


Keeping things private:

Your name, address and birthday are put on a computer.
We will keep things you tell us private.
Sometimes we will have to tell other people who are working with you, like your doctor or social worker. We will ask you first.


Where we keep things we have written about you:

  • At the Day Centre
  • Where we work
  • In your home
  • Some things will be in a filing cabinet
  • Some things will be kept on a computer


What you can see:

You can see nearly everything we write about you.
There are some things you might not be able to see.
You can ask when we are writing things down.
You can ask us to explain things.
We will talk to you about what we write in letters if you want us to, we will usually give you a copy.


If you want to read what has already been written about you:

Ask us and we might be able to show you and read it with you. If we cannot show you, you will have to fill out a form and post it to the Community Team Manager.

You can ask someone to help you:

  • Your social worker
  • Your family
  • Support staff
  • Your Advocate


You might have to wait:

You might have to wait a little while to read what is written about you. You might have to wait up to 40 days.

Someone can help you read what you have asked to see.

  • Your family
  • Your support staff
  • Your Advocate


If we get things wrong:

If we write things that are wrong we will change them.


If you are still not happy:

You can talk to your social worker or advocate if you are still not happy. They will help you.
You can make a complaint.


You can phone your Social Worker.
You can ask someone to help you phone the right number.
