Your Assessment Your Community Care Assessment

Community Care Assessment works out what support you need.

Your Social Worker will do your Community Care Assessment.


Your Social Worker Will:

  • Talk to you
  • Find out what you think
  • Talk to people who know you well. We will ask you first
  • Write down what we know about you
  • Write down the support you need
  • Show you what we have written to make sure we have got it right

At the end of the Assessment a list is written. This is called a Care Plan.


Your Care Plan:

A Care Plan is a list of what you need and people who will support you.
Your Social Worker will make the Care Plan with you by:

  • Finding out what support Social Services can give
  • Working out who else can support you


Your Social Worker Will:

  • Help you understand by using symbols, pictures and Easy To Use Words
  • Send you a copy of your Care Plan
  • Send a copy of your Care Plan to the people who are going to support you
  • At your Review ask you if your Care Plan is still ok

Community Care Assessment and Care Plans will take a long time to do. If you need support straight away you can have a quick Community Care Assessment. This will take one week.

Your Social Worker will do a new Community Care Assessment when things change.

You can ask someone to help you phone the right number.
