Your Review
At your review you can talk about:
- How you are getting on
- The support you get
- What is going well
- What is not going well
We could talk about:
- Your home
- Family and Friends
- Things you do
- Your money
- Other things you want to do
You will have a Review every year
Getting ready for your Review
Someone who knows you well will help you get ready for your Review.
They will talk to your Social Worker.
You can choose:
- Who you want us to talk to
- Where you have your Review
- When you have your Review
- Who comes to your Review
At your Review
- You can say what you think
- You can choose who supports you at your Review
- Your Social Worker will help you by using Easy to Use Words
- Your Social Worker will write down what people say
After your Review
Your Social Worker will send you a list called a Care Plan.
Your Care Plan will say:
- What we talked about
- What support you need
- Who will support you
- How to contact your Social Worker
- When your next review will be
You can say who has a copy of your Care Plan
Someone who knows you well will talk about your Care Plan with you.
You can phone your Social Worker
You can ask someone to help you phone the right number.
