The South West region of Parent Carer Forums have launched a survey for parent carers of children and young people with Special Educational Needs and/or Disabilities (0-25). Along with NHS England they are working to better understand the experience of families.
The survey will focus on the experience families have during a transition, such as:
· moving to a new class/education setting
· changes in a service such as a new department or worker
· moving from children to adult services
The information you provide is anonymous and you can complete the survey more than once if you would like to tell us about more than one transition. Please visit www.surveymonkey.co.uk/r/SWtransitions to access the survey.
The responses will be used in a report due to be published early in 2023 which will highlight areas that can be improved and things that are supportive and helpful for children and young people.
At the end of the survey, you will be given the opportunity to enter a free prize draw. Prizes include:
