This is an update for families, young people and professionals about Education, Health and Care Plan (EHC plan) phase transfers for this academic year. 

There are more phase transfer cases than usual this year, and fewer staff available to manage them. Because of this, it will not be possible to meet the usual deadlines for every child and young person, despite teams continuing to work through cases as quickly as they can. 

The priority is to make sure every child and young person moving to a new stage of education is offered a placement that meets their needs. Work is ongoing with parent carers, schools and partner organisations to find the right placement for each individual. 

Placements are being confirmed as soon as possible so families and education settings can plan ahead, including time for visits where possible. 

The EHC plan pathway on the Somerset SEND Local Offer has been updated with clearer, stepbystep information, clearer headings and current average waiting times. Information about phase transfers has also now been added, and there is increased use of online forms, with paper options still available for those who need them. 

Updates will continue to be shared, and families will be contacted directly when there is information about an individual placement.
