An updated checklist is now available to download for schools to use when requesting an Education and Health Care Needs Assessment (EHCNA).

Created by a multi-agency working group including Educational Psychologists, representatives from the SEND team, Learning Support Team and Somerset Parent Carer Forum, as well as school-based SENCos, it has been designed to make the process easier.

Further details about the process can be found on the SEND Professional Portal.  The portal is used by Somerset Council’s Statutory SEND Team as a resource to gather the key information needed around an Education, Health and Care (EHC) Assessment or an EHC Plan Annual Review.
