Preparing for adulthood and transitions

Learn how Somerset helps children and young people with SEND prepare for adulthood and build independence

About preparation for adulthood

Preparation for adulthood is an ongoing process that starts in the early years and continues through education and into adult life.

It should help children and young people to:

  • Develop independence and life skills
  • Build confidence and self-advocacy skills
  • Take an active part in their learning, community and relationships
  • Prepare for employment, further education and community participation

Supporting successful transitions

Transitions can be important points of change in a child or young person’s life. Effective planning helps ensure continuity of support and positive outcomes.

All transitions should be:

  • Planned early and managed in a timely way
  • Centred on the child or young person’s strengths, needs and aspirations
  • Developed in partnership with families, education settings and professionals
  • Supported by clear communication and effective information sharing
  • Focused on helping children and young people feel prepared, confident and included

Preparation for adulthood should be considered at every stage of the graduated response, helping children and young people develop the skills and confidence they need for adult life.
