Overview
When you reach 14 years old, the service you are involved with will start to talk to you about a ‘transition’ care plan and what transition will mean for you. This will depend on a number of things, such as your current skills, abilities and needs, as well as the nature of your underlying health condition.
Childhood to adult charter
A charter is something that tells you what you can expect us to do. Young people have created this charter to help other young people. It’s here to help services support young people as they grow up and face changes in their lives.
Understanding changes
Some aspects of your care will not need to transition but some will. We will work with you to start talking with the adult services that may be involved in your care so that when you reach 18 your transition of care will be easier and you will have been involved in each stage of the process.
