Description

We complete strengths-based assessments for young people who may require support from Adult Social Care in adulthood under the Care Act (Care Act Assessments) focused on the outcomes they want to achieve. These Strength-based assessments should be Person-Centred and focus on the 4 Preparing for Adulthood Outcomes:

  • Employment
  • Independent living
  • Community inclusion
  • Health

We want to make the Transition into Adult Social Care is as smooth as possible and ensure appropriate care and support is in place for when the young person turns 18 years old.
