You told us
Young people and families said:
- The team name was confusing
- They did not know what help Adult Social Care gives at 18
- They wanted clear information about moving to adult services
- Planning and talking to services needed to start sooner
- Children’s and Adult Social Care should talk to each other more
The problem
Moving from children’s services to adult services is a big change. People told us it wasn’t clear who does what, when help starts, or how to get support. This made things stressful and harder than it should be.
We are listening
We’ve made changes based on what you said:
- New name: We changed our name to the Unstoppables’ suggestion of Transition to Adult Social Care Team.
- Better information: We updated the Local Offer, created a leaflet, and are developing an animation with The Unstoppables.
- Earlier support: We now allocate workers to young people from age 16 or 17 and do Care Act assessments earlier.
- Improved referrals: We made an online referral form and new ways to pick up referrals quickly.
- Joint working: We run workshops and meetings with Children’s Social Care and schools to plan earlier.
- Tracking progress: We built a Transition Dashboard so teams can see where young people are in the process.
What happens next
We’re still making improvements:
- An easy-read leaflet for young people
- A follow-up session with The Unstoppables to check progress
- A joint policy for transition and new planning and decision making forums from January 2026
