About

information

This pathway is for children aged 0 to 5 years who are not yet in school.
For those aged 4 but in school please see the Assessment Pathway for 5 to 17 year olds (schools).

If there are concerns that a child aged 0 to 5 may be autistic, a professional can refer them to Somerset Foundation Trust (SFT).

The GP, health visitor, Speech and Language Therapist or Occupational Therapist should use the Advice and Guidance route. If this is not possible, they can email the Community Child Health (CCH) secretaries at CCHadmin@somersetFT.nhs.uk.

The referral must include detailed information. This should cover a developmental assessment, nursery reports, and any speech and language reports.

Assessment

If the referral is accepted, this is what happens next:

  • Initial contact
The service that contacts you depends on your child’s age and your GP practice area.
  • First assessment
Your child will have an appointment for an initial assessment.
  • Further assessments
More appointments may be needed. These involve the right professionals and are based on your child’s needs.
  • Diagnosis
Clinicians review all the information and decide whether an autism diagnosis is appropriate.
  • Sharing the outcome
The outcome is shared with you and your child in a face-to-face appointment. You also get a written report.

An autism diagnosis may or may not be given. Whatever the outcome, you will be signposted to relevant support.

Support

Portage Workers and Transition Workers from the Virtual School can support families when autism is identified early, or while a child is going through the assessment pathway. They do this by offering clear advice, guidance and signposting to other services. They may run focus groups, stay and play sessions, or give direct support. Some children may also get support at home through visits.
