A continuous cycle of support
Work with children, young people and families wherever possible. Their views, experiences and aspirations should help shape decisions about support and provision.
The graduated response uses a continuous cycle of Assess, Plan, Do and Review. This helps settings understand needs, provide effective support and review the impact of that support.
The cycle helps to make sure support is:
- responsive to changing needs
- developed through collaboration with children, young people and families
- focused on positive outcomes
Support should remain flexible. Children and young people may move between different levels of support as their needs change and outcomes are achieved.

Assess
Gather information to build a clear understanding of the child or young person.
This includes:
- identifying strengths, needs and barriers to learning
- using assessment, observation and evidence from practice
- listening to the views of children, young people and families

Plan
Agree what support will be provided and what outcomes you want to achieve.
This includes:
- agreeing outcomes with children, young people and families where appropriate
- deciding which strategies and interventions to use
- setting clear timescales and review dates
- identifying who will provide support and how often

Do
Put the agreed support into practice.
This includes:
- delivering high-quality teaching and inclusive practice
- providing targeted support where needed
- monitoring implementation and progress

Review
Evaluate the impact of support and decide on next steps.
This includes:
- reviewing progress towards agreed outcomes
- gathering feedback from children, young people and families
- considering what has worked well and what needs to change
- adapting support to meet current needs
