The pre-assessment pathway documents for Autism and ADHD can be found on this page. These documents have been developed in collaboration with education, health and care colleagues as well as parent carers, children and young people. They have been developed to support education settings, practitioners, and parent carers so that everyone better understands the pre-assessment pathway.

This document is designed to support education settings, practitioners and parent carers so that everyone better understands the pre-assessment pathway.
Services that support children and young people at the earliest stage without a need for a diagnosis include:

- Family Intervention Service (FIS)

Pre assessment pathway for possible autism and ADHD interventions graduated response. The purpose of this document is to outline possible interventions, which after careful screening for special education needs, should be implemented immediately rather than wait for a possible diagnosis.
WISE Up Workshops
Being a parent carer of a child or young person with additional needs can bring extra situations which are challenging to know how to handle. To help support parent carers navigate these a range of workshops have been created by Somerset Parent Carer Forum based on the subjects you have told them would be helpful.
The idea behind the project is to empower parent carers through Workshops for Information, Support and Education. All the workshops are delivered or co-delivered by someone with lived experience of being a parent carer.

Next Steps Form
If evidence is gathered that indicates a child or young person may have either Autism and/or ADHD a ‘Next Steps – Autism and ADHD Pathway’ form should be completed to request an assessment for Autism and/or ADHD. The guidance of how to complete this process is within the document titled: Autism and ADHD Pre-assessment Pathway Guidance.
Form to provide information to inform whether a child or young person needs a diagnostic assessment in relation to Autism and/or Attention Deficit/Hyperactivity Disorder (ADHD). This form contains sections to be completed by a referrer (SENCO, health or social care practitioner, PFSA), parent and child/young person. Please download and save to your computer, and use this form in Word and not in your web browser.
If you would like support in completing this form, we would advise you to ask whoever you think is best placed to help. This might be someone at your child’s school (e.g. SENCo), or someone else who is working with you (e.g. PFSA, Social Worker or health practitioner).
